By submitting this Channel Information Form, you confirm that you are submitting your Television Network (or Television Channel) for broadcast consideration to become a part of a Family of Television Networks that broadcast on Z Super Channel. If my channel is approved for broadcast on Z Super Channel, I confirm and understand that I will be billed ($250.00) Two Hundred Fifty Dollars each month for the monthly programming, storage and maintenance costs for my Channel (that is named above and contained herein this Channel Information Form). By submitting this Channel Information Form and your Channel Logo, you acknowledge that an Official Representative of Z Super Channel will notify you via email within 5 to 10 days of the status your TV Network Broadcast determination. If your Television Network is accepted for broadcast on Z Super Channel, we will forward to you a Written Agreement to sign and return along with your first monthly payment. Your channel will be ready for daily broadcast on Z Super Channel within (30) Thirty Days of our receipt of all your requested documents and your payment.
