This form is for Venue Owners & Promoters to Book the taping and production of "The After Show Live At .... (Your Venue's Name)" or to book various other Z Network Channel Live! Events at your Venue for television broadcast on a Z Television Network.

This form below is to be used when you are prepared to make an OFFICIAL OFFER to Z NETWORKS / FIRST POWER MEDIA GROUP to host a Live Television Show Taping & Performance Event at your Venue. Please do NOT submit this Offer Form before speaking with an Authorized Agent of Z NETWORKS/FIRST POWER MEDIA GROUP regarding the Show Fee and availability. Once this form is received it is considered an Official Offer and will be submitted for approval. If accepted, a performance contract will be issued to the Purchaser and the Purchaser agrees to abide by all of the terms and conditions on the Contract and Riders. The person submitting this offer and Venue Booking Form MUST be at least 21 years of age. Thank you.
